William L. Self, DNP, MSN, CRNA, ARNP

Co-Founder, Life is Ketaful

Dr. Self has well over ten years of experience working in the Volusia County area as a ICU and Advanced Practice nurse. Dr. Self is a lifelong Volusia County resident and United States Army veteran. He graduated from Daytona Beach Community College with both AA and nursing degrees before heading to the University of Central Florida to obtain his Bachelors of Science in Nursing. After spending several years in the Trauma/Surgical ICU, Dr. Self returned to school at the University of North Florida where he obtained both a Masters of Science in Nursing-Nurse Anesthetist Program and Doctorate of Nursing Practice. Currently, Dr. Self is an Instructor in the University of North Florida Nurse Anesthetist Program and maintains a clinical practice in Daytona Beach.

Because of Dr. Self’s strong ties to his hometown, offering treatment for depression and pain for Central Florida residents is one of his most important professional priorities. Dr. Self has said “depression can interfere with normal family dynamics, so providing ketamine treatment is crucial to return families back to a sense of normalcy and happiness”.

Dr. Self is a member of the American Association of Nurse Anesthetist (AANA) and Florida Association of Nurse Anesthetist (FANA).

Sadye Dexter, MSN, APRN, FNP-C

Sadye is joining the Life is Ketaful family adding Primary Care services as a board-certified Family Nurse Practitioner. She has over 10 years of nursing experience where she has worked in both intensive care, cardiovascular intensive care units, and most recently in Family Practice. She received her Associate and Bachelor of Science in Nursing at Daytona State College and received her Master of Science in Nursing with a focus in family practice at Chamberlain College of Nursing. Sadye’s practice philosophy involves individualized, patient-centered care with a focus on wellness, quality of life, and personal health care goals.

Sadye and her husband Craig have been together for 11 years and have 4 busy and beautiful children who this year are 4, 8, 9, and 16. When she is not working, Sadye enjoys spending time outdoors when possible with family and friends as well as creative time in her sewing room.

Sadye is very passionate about mental health and is honored to work side by side with the Life is Ketaful team to help treat a variety of mental health diagnoses to return patients to a desired quality of life.

She is a member of the Florida Association of Nurse Practitioners (FLANP) and the American Association of Nurse Practitioners (AANP)

April Verga, RN

April has been a registered nurse since 1998 and has a background in Labor and Delivery and IV hydration. Working with patients is her passion. April believes this office implements best practice in the use of Ketamine for recovery from depression, PTSD and pain. She also believes in the benefits of IV hydration and vitamin nutrition. April heard the call and has now found a home here at Life is Ketaful.

Carlos N. Montalvo M.D.- Anesthesiologist

Medical Director and Co-Founder, Life is Ketaful

Dr. Carlos N. Montalvo is a graduate from Stony Brook Medical School in New York. He completed Residency training in Anesthesiology at Saint Barnabas Medical Center in New Jersey. Dr. Montalvo is now a Anesthesiologist in Daytona Beach, Florida. During his training and now currently in clinical practice, he was fascinated by the beneficial effects of ketamine on patients with chronic pain during operative and postoperative period. He has become a sponsor of Ketamine IV infusions for outpatients after reviewing the literature supporting its efficacy and witnessing its profound effects on depression and chronic pain patients.

Dr. Montalvo is a member of American society of anesthesiologist (ASA) and Florida Society of Anesthesiology (FSA).
